This form begins the process of enrolling in CHAMPS with our agency. This is a state-required enrollment and a caregiver must be approved before they are allowed to begin providing services.

If you are a caregiver living with your client, you will need to fill out this form and submit it to MDHHS in order to be exempt from the Electronic Visit Verification (EVV) requirement. It certifies that you live with your client and will need to be filled out at every 6 month review.
